If  you are allowing your minor child to race at a Cycle City Promotions event without you present, the following documentation is required ( a minor is anyone under 18 years of age):

1.       A notarized letter (see sample letter under the “forms” link on our website)

2.      A completed minor release form (print form from our website under “forms” link)

3.      A completed entry form (print from our website under “forms” link)

4.      A copy of your driver’s license and insurance card

If you have a different last name than the minor child you are entering you will need some type of proof that the child is yours.  Examples might be:

1.      An insurance card with your current last name and the minors name

2.      A notarized letter from a parent or guardian with the same last name as the child, stating that you are the legal guardian of the child.  You may want to make multiple copies and bring to each race as we will require a copy each time you enter the minor.

3.      A copy of the minor’s birth certificate (this will work if the mom will be entering the child but has a different last name now.  The birth certificate will show the mom’s first and middle name and that along with a current driver’s license showing those same names will suffice)

Your cooperation is greatly appreciated.  No excetions will be made to this policy.
